How To Register

If you are interested in registering with this practice, you will need to either download the application forms or alternatively collect them from the practice.

To register with this practice you need to:

• Complete the applications form provided by the practice including the GMS1 form.sperate forms will need to be completed for each person wishing to register with the practice.

• All new resgistering patients will need to live within the Northampton Borough area, some of the districts are mentioned below but please refer to map below for more details.

• Provide proof of address (Such as Utility Bills, Council Tax Statements etc. This can be no older than 3months. Mobile phone bills are NOT accepted)

• Provide proof of identity (photo identity is essential e.g. Valid Passport Or Driving Licience)

• When Registering Babies parents must provide a Birth certificate or the Red Book you were given when discharged from Hospital.

• When Coming from Non-EU Contries please provide original Visa, Passport and proof of current address. If the Home Office is holding documentation we advise you request a letter of explanation.

• All Visa Documentation should be valid for a minimum of 6 months, this includes resident permits, work permits,working holiday visa, joint spouse visa, student visa  and  letter from the establishment
